Haemodynamic effects of moderate normovolaemic haemodilution in conscious and anaesthetized patients.

چکیده
We have assessed the haemodynamic effects of moderate normovolaemic haemodilution in ASA I patients, either conscious or during enflurane-fentanyl anaesthesia (10 patients in each group). Cardiac index (CI), stroke index (SI) and ejection fraction (EF) were measured by transthoracic electrical bioimpedance and, in the anaesthesia group, arterial and central venous blood samples were obtained to assess oxygen delivery (DO2), oxygen consumption (VO2) and oxygen extraction ratio (O2ER). In conscious patients, heart rate (HR) remained unchanged as SI, EF and CI increased. When haemodilution was performed during anaesthesia, CI remained stable in spite of a slight increase in SI, as HR decreased. This produced a reduction in DO2 which was compensated for by an increase in O2ER, allowing maintenance of VO2 without alteration in blood lactate concentration.

Comparative effects of induced hypotension and normovolaemic haemodilution on blood loss in total hip arthroplasty.
The effects of deliberate hypotension and normovolaemic haemodilution on operative blood loss were studied in patients undergoing hip arthroplasty. Thirty patients were anaesthetized with halothane-nitrous oxide, and assigned to one of three groups. In group I, mean arterial pressure was decreased to 55 mm Hg by sodium nitroprusside. In group II, 23 +/- 2 ml/kg of blood was withdrawn just befor...
متن کاملHydroxyethyl starch is superior to lactated Ringer as a replacement fluid in a pig model of acute normovolaemic haemodilution.
BACKGROUND Tissue tolerance to oxygen privation during acute normovolaemic haemodilution with different fluids remains unclear. We tested the hypothesis that hydroxyethyl starch (HES) is superior to lactated Ringer's solution in pigs for preserving tissue perfusion during acute normovolaemic haemodilution. METHODS Twenty-four animals were randomized into control, lactated Ringer's solution an...
متن کاملRegional blood flow in normovolaemic and hypovolaemic haemodilution. An experimental study.
The effects on the circulation of limited normovolaemic haemodilution with dextran 70 and subsequent haemorrhage to a mean arterial pressure of 60 mm Hg were studied with isotope-labelled microspheres in the dog. Following haemodilution, cardiac output, stroke volume and systemic oxygen transport increased. The distribution of oxygen to the heart, liver (hepatic artery), spleen and carcass (mai...
متن کاملRheological properties of commonly used plasma substitutes during preoperative normovolaemic acute haemodilution.
Preoperative normovolaemic acute haemodilution (PNAH) is used to reduce major blood loss during elective surgery. Considerable attention has been paid to colloid osmotic pressure, index of diffusibility and intravascular half-life of the currently available substitutes, but there is little information on their rheological properties from in vivo studies. Forty patients undergoing elective aorti...
متن کاملAcute normovolaemic haemodilution--is it a solution to reduce perioperative blood transfusions?
Haemodilution is a technique used to reduce perioperative homologous blood transfusions. Haemodilution is a poorly investigated technique in veterinary medicine. This article reviews haemodilution as a potential technique to reduce perioperative homologous blood transfusions. The history of haemodilution is briefly reviewed followed by the mathematical basis to haemodilution. The issue of criti...
